0592T: the health coaching follow-up session

How CPT® 0592T works under the CY 2027 Medicare Physician Fee Schedule Final Rule, the add-on question nobody has answered yet, and what to document in each follow-up.

By Angel Reyes, MCHES®, MPH, DrPH(c)Last verified: November 5, 2026

Short answer

0592T is the Category III CPT code for an individual, face-to-face health and well-being coaching follow-up session of at least 30 minutes. It is nationally priced from January 1, 2027, furnished by a qualified coach under general supervision, and billed only by the supervising practitioner. CMS lists it with an add-on ("ZZZ") indicator, and how contractors will process a follow-up billed without 0591T is still unresolved.

The open question: 0592T's add-on indicator

Addendum B gives 0592T a global indicator of "ZZZ", the PFS code for an add-on service reported with a primary service. That mirrors its crosswalk code, 99489, which is an add-on to 99487. The rule text does not explain the indicator or say how a follow-up session billed on a day without 0591T will be processed.

Until your Medicare Administrative Contractor publishes guidance, treat this as unresolved. Ask your billing team how they plan to report standalone follow-ups, and watch the Sources & updates page for changes.

Documenting a follow-up session

The rule's coaching section lists no coaching-specific documentation elements. This workflow follows general incident-to and time-based documentation practice; your supervising practitioner's and organization's policies come first.

  1. Time: record actual start and stop times. The descriptor requires at least 30 minutes.
  2. Subjective: what the patient reports about progress on each SMART goal since the last session, and new barriers.
  3. Objective: modality, measures the patient tracked, and attendance.
  4. Assessment: progress relative to the practitioner's plan of care, within coaching scope.
  5. Plan: adjusted goals, the next session, and any new clinical issue referred back to the practitioner. The coach does not triage.
  6. Sign-off: coach name, credential and signature; supervising practitioner named.

Fictional example (annotated)

Note type: Health coaching follow-up · Modality: in person · Time: [actual start]–[actual stop]

S: Walked after breakfast 3 of 4 planned days. Missed day after a double shift. Reports knee soreness on stairs.

O: Patient's step log reviewed.

A: Steady progress toward the walking goal. Knee soreness is a new symptom, outside coaching scope.

P: Keep 4 days/week; plan a shorter walk on double-shift days. Knee soreness referred to Dr. [name] per protocol. Next session in 2 weeks.

Notice the referral: a new symptom goes back to the practitioner rather than being assessed by the coach. Bracketed fields are placeholders; never pre-fill time or outcomes.

Same-day code pairs to check

CPT parenthetical guidance indicates 0592T should not be reported with 96156, 96158, 96159, 98960, 0488T or 0591T. Verify these against the current AMA CPT® code set and your MAC's edits before relying on them.

Verification checklist before billing

National pricing with status A means 0592T is payable under the fee schedule. It does not promise payment for any particular claim. Before the first claim, the practice should confirm:

  • The supervising practitioner's order or plan of care supports continued coaching, and the note links to it.
  • The coach's credential file is current, including evidence of coaching-specific training.
  • Actual session time meets the 30-minute descriptor and is recorded.
  • How your MAC will process 0592T given its ZZZ indicator, especially for follow-ups billed without 0591T.
  • Same-day code pairs and National Correct Coding Initiative (NCCI) edits.
  • Telehealth modality, place-of-service and modifier requirements, if the session was remote.
  • Beneficiary cost-sharing treatment. The rule does not address it, so confirm with your MAC.
