Who furnishes and who bills
Under the CY 2027 Medicare Physician Fee Schedule Final Rule, health and well-being coaching services under CPT® 0591T–0593T are furnished by qualified auxiliary personnel under the general supervision of, and billed by, an eligible Medicare practitioner (42 CFR 410.26(b)(5)). CHES®, MCHES®, NBC-HWC, and other coaches are not independent Medicare billers.
Your supervising practitioner and organization come first
Always follow the documentation, billing, and scope-of-practice policies established by your supervising practitioner, employer, and compliance department. The templates and examples on this site and in the toolkit are starting points and must be adapted to their requirements.
Those policies must also comply with current CMS regulations, your Medicare Administrative Contractor (MAC), payer contracts, and state law. If anything here, or any instruction you receive, appears to conflict with those requirements, stop and consult your compliance officer or qualified healthcare counsel before billing. A supervisor's instruction does not make a noncompliant claim compliant.
No guarantee
Purchasing or using the toolkit does not guarantee eligibility, credentialing, coverage, payment, or reimbursement. Payment figures shown on this site are estimates calculated from CMS Addendum B relative value units and the rule's estimated conversion factor. They are not CMS-published amounts, vary by locality and setting, and are paid to the billing practitioner.
