Health Coach Codes is written by a Master Certified Health Education Specialist and former military medic. It explains health and well-being coaching CPT® codes 0591T, 0592T and 0593T in plain English, with every rule claim traced to its source.
Why this site exists
On November 4, 2026, CMS released the CY 2027 Medicare Physician Fee Schedule Final Rule (CMS-1848-F). It set national payment for health and well-being coaching codes 0591T, 0592T and 0593T, effective January 1, 2027. It also named "National Commission for Health Education Credentialing eligibility for Certified Health Education Specialists" as one of the certification pathways for the auxiliary personnel who furnish the service.
Most guidance on these codes was written before the final rule. Very little of it speaks to health education specialists or to the practices that supervise them. Health Coach Codes fills that gap with a field-manual approach: clear procedures, nothing assumed, and every claim cited.
What a medic's training brings to documentation
Field medicine runs on standard procedures, precise records and clear handoffs to the next level of care. The same discipline applies here. Coaches document; the supervising practitioner bills. A coach never triages; new clinical issues go back to the practitioner. The veteran-care annex draws on that service directly, for civilian coaches working with veteran patients.
How content is researched and verified
- Primary sources first. Rule claims are checked against the CMS-1848-F display copy, Addendum B and the regulation text at 42 CFR 410.26(b)(5). Page citations are given where possible.
- Fact versus open question. Content separates what the final rule says from what it does not address, such as MAC processing of 0592T's add-on indicator. Open questions are labeled as open.
- Derived numbers are labeled. Payment estimates are calculated from Addendum B RVUs and the rule's estimated conversion factor. They are marked as estimates, not CMS-published amounts.
- Dated and maintained. Every page shows a "Last verified" date. Content is reviewed when the Federal Register version publishes, when MACs issue guidance, and when NCHEC or CMS release updates. Changes are logged on the Sources & updates page.
- Plain claims only. The site never says a credential, template or product is "CMS-approved", and never implies a coach can bill Medicare independently.
