0593T: group health coaching, billed per beneficiary

How CPT® 0593T works under the CY 2027 Medicare Physician Fee Schedule Final Rule, and how to document a group session so every participant has an individual record.

By Angel Reyes, MCHES®, MPH, DrPH(c)Last verified: November 5, 2026

Short answer

0593T is the Category III CPT code for face-to-face health and well-being coaching in a group of two or more people, at least 30 minutes. It is billed once per beneficiary in the group (rule p. 375). From January 1, 2027 it is nationally priced; a qualified coach furnishes it under general supervision, and only the supervising practitioner bills.

How per-beneficiary billing works

One group session produces one 0593T claim line for each Medicare beneficiary who attended, each billed by the supervising practitioner for that patient. That makes every participant's record matter on its own. A group note copied across the roster does not show what happened for each person.

Some commenters asked CMS to allow multiple units of 0593T for longer group sessions (rule p. 385). The final rule does not explicitly rule on multiple units. Follow the current CPT® descriptor and your MAC's guidance.

Documenting a group session

The rule's coaching section lists no coaching-specific documentation elements. This workflow follows general incident-to and time-based documentation practice; your supervising practitioner's and organization's policies come first.

  1. Roster: list each participant, and note which are Medicare beneficiaries with a practitioner order on file.
  2. Group record: topic, total group duration (start and stop), modality and number of participants.
  3. Individual entry for every beneficiary: their participation, their own goal progress, and their next step. Never clone one entry across the group.
  4. Referrals: any new clinical issue for a participant goes back to that participant's practitioner.
  5. Sign-off: coach name, credential and signature; supervising practitioner named for each beneficiary.

Fictional example: one beneficiary's entry

Group: "Moving more with arthritis" · 6 participants · Time: [actual start]–[actual stop] · in person

Participant: [initials/ID] · Participation: shared a barrier (cold mornings), practiced the chair-stand routine with the group.

Progress: reports 3 short walks this week toward a 5-walk goal.

Plan: try an indoor mall walk on cold days; review next group session. No new clinical concerns.

The other five participants each get an entry written about them. Bracketed fields are placeholders; never pre-fill time or outcomes.

Same-day code pairs to check

CPT parenthetical guidance indicates 0593T should not be reported with 96164, 96165, 97150, 98961, 98962 or 0403T. Verify these against the current AMA CPT® code set and your MAC's edits before relying on them.

Common 0593T documentation mistakes

  • One note for the whole group. Each billed beneficiary needs an individualized entry.
  • No roster or attendance proof. Record who attended, and for how long if someone left early.
  • Missing total duration. The descriptor requires at least 30 minutes; record actual start and stop.
  • Assuming extra units for long sessions. The rule gives no explicit ruling on multiple units.
  • No practitioner order for a participant. Each beneficiary's coaching should carry out that patient's practitioner's plan.
  • Unverified telehealth groups. If the group meets remotely, confirm modality and place-of-service requirements with your MAC.

Frequently asked questions

Can 0593T be billed for groups?

Yes. 0593T is specifically the group code, for two or more individuals, and it is billed once per beneficiary in the group.

Can I bill multiple units of 0593T for a long group session?

The final rule does not explicitly address multiple units. Follow the current CPT® descriptor and your MAC's guidance.

Do non-Medicare participants affect billing?

0593T claims are submitted for the Medicare beneficiaries who attended. Other participants' coverage depends on their own payers' policies.

How much does Medicare pay for 0593T?

About $22.51 per beneficiary, national non-facility (estimate from Addendum B RVUs; not a CMS-published figure), paid to the billing practitioner.
