Terms of use

By using healthcoachcodes.com or the Health Coach Codes toolkit, you agree to these terms.

Effective: November 5, 2026

healthcoachcodes.com and the Health Coach Codes toolkit are operated by Subthesis ("we"). Contact: Angel@subthesis.com.

1. Educational use only

All content is educational and is not legal, billing, coding, reimbursement or clinical advice. The full disclaimer is part of these terms. You are responsible for following your supervising practitioner's and organization's policies, CMS regulations, your Medicare Administrative Contractor's guidance, payer contracts and state law.

2. Purchases

The toolkit is sold through Teachable. Price, delivery and refund terms are shown on the checkout page at the time of purchase, and Teachable's terms also apply to the transaction.

3. License

When you buy the toolkit, you receive a personal, non-transferable license to use and adapt the templates in your own work or within the organization you work for. You may not resell, redistribute or publish the toolkit, or share your access with others. Team licenses, if offered, cover the number of seats purchased.

4. Templates require adaptation

Templates are starting points. They must be adapted to your organization's policies, EHR and the individual patient encounter. Do not pre-fill times, medical-necessity language or outcomes. Any contract or agreement template must be reviewed by a qualified attorney before use.

5. Intellectual property

Site content, the toolkit and the Health Coach Codes name are owned by Subthesis, except third-party material identified as such. CPT® is a registered trademark of the American Medical Association, and code descriptors are paraphrased. Short quotations from public CMS documents are cited to their source.

6. No warranty

Content is provided "as is." We work to keep it accurate and dated, but rules, contractor guidance and payer policies change, and we do not warrant that content is complete, current or suitable for any particular claim or situation.

7. Limitation of liability

To the fullest extent permitted by law, Subthesis is not liable for indirect, incidental or consequential damages, or for claim denials, recoupments, penalties or other losses arising from use of the site or toolkit. Our total liability for any claim relating to a purchase is limited to the amount you paid for it.

8. Changes

We may update these terms. The effective date above shows the current version, and material changes are noted on the Sources & updates page.

9. Governing law

These terms are governed by the laws of the State of California, without regard to conflict-of-law rules.
