Chronic care management vs health coaching: how CCM and 0591T–0593T differ

CCM and health coaching both use clinical staff under general supervision, and the 2027 coaching values are even crosswalked to complex CCM. They are still different services. Here's how they compare for practices running both.

Practices that already run chronic care management (CCM) are asking how the 2027 health coaching codes fit alongside it. The two are related. CMS crosswalked the final values for 0591T and 0592T to complex CCM codes 99487 and 99489 (CMS-1848-F, p. 379). But they are separate services with different purposes, eligibility and documentation.

CCM requirements summarized from prior Physician Fee Schedule rules and CMS CCM guidance; confirm current requirements with CMS and your MAC before billing.

Can the same patient receive both?

The final coaching rule doesn't prohibit it, and it doesn't address how coaching and CCM interact. The core principle to apply with your billing team: the same minutes can't be counted twice. Coaching session time documented for 0592T shouldn't also be counted toward a CCM monthly time threshold. Check same-day and same-month edits, and ask your MAC how it will handle patients receiving both.

Can a health coach perform CCM activities?

CCM is furnished by clinical staff under general supervision. Whether a particular coach counts as clinical staff for CCM depends on CMS's incident-to definitions, state scope rules and your organization's policies, not on their coaching credential. Treat it as a separate question from whether they can furnish 0591T–0593T.

Documentation: keep the services distinguishable

  • Write coaching sessions as coaching notes, with session start and stop times, and CCM activities in the CCM log with its monthly time tally.
  • Link both to the same practitioner plan, but never reuse text across them.
  • When a coaching session surfaces a care-coordination need, refer it to the CCM team and note the referral.

Why the crosswalk matters

CMS valued 0591T and 0592T by crosswalking work RVUs, work times and direct practice-expense inputs to 99487 and 99489, which "account for more time, specifically, 60 minutes of clinical staff time and each additional 30 minutes" (p. 379). That tells you how CMS views the effort of coaching, but it does not turn coaching into CCM or import CCM's requirements.
