Health coaching's road to Medicare: from Category III codes in 2020 to national pricing in 2027
A dated timeline of how health and well-being coaching codes 0591T–0593T went from emerging-service codes to nationally priced Medicare services, and what to watch next.
Health coaching didn't arrive in Medicare overnight. The codes CMS priced nationally on November 4, 2026 have existed for years. Knowing that history helps explain why the final rule looks the way it does, and why some questions are still open.
The timeline
| When | What happened |
|---|---|
| Jan 1, 2020 | CPT Category III codes 0591T, 0592T and 0593T take effect for health and well-being coaching. CMS's rule notes the codes "have been in effect since January 1, 2020" (CMS-1848-F, p. 378). |
| 2020–2026 | Medicare payment is contractor priced: each Medicare Administrative Contractor decides whether and how much to pay. |
| CY 2024 PFS | CMS adds the coaching codes to the Medicare Telehealth Services List (88 FR 78859–78860, cited at rule p. 384). |
| Summer 2026 | The CY 2027 proposed rule offers national valuation, direct supervision, and work RVUs of 1.00 / 0.70 / 0.23, and names credential pathways including NCHEC CHES® eligibility. |
| Nov 4, 2026 | The final rule is displayed: general supervision, higher values for 0591T (1.81) and 0592T (1.00), Category III codes kept, G-codes declined. |
| Nov 9, 2026 | Scheduled Federal Register publication. |
| Jan 1, 2027 | National payment (status A) takes effect. |
Why the history matters
Category III codes signal "emerging." Category III codes track new services and procedures. CMS chose to price them rather than create HCPCS G-codes, saying the CPT codes and prefatory language "are recognized nationally" (p. 386). That keeps the coaching definition, including its training-program language, tied to the CPT code set.
Six years of contractor pricing left uneven habits. Some practices never billed these codes; others had local experience. National pricing resets the baseline, but MAC processing details, like how 0592T's add-on indicator will work, still come from contractors.
The proposal-to-final change was large. Supervision moved from direct to general, and two of three values rose after public comment. Content written between the proposed and final rules is now out of date in important ways. Check the date on anything you read, including ours (every page shows a "Last verified" date).
What to watch next
- Federal Register citations replacing display-copy page numbers.
- MAC articles on documentation, the 0592T ZZZ indicator, and telehealth billing details.
- NCHEC guidance on how the CHES® eligibility pathway is interpreted.
- Utilization monitoring. CMS said it will monitor use and may revisit frequency in future rulemaking.
We log every change on the Sources & updates page. For the full proposed-vs-final comparison, read what the 2027 final rule changed.
