How do CHI codes G0019 and G0022 differ from coaching?

Answered by Angel Reyes, MCHES®Last verified: November 5, 2026

Short answer

G0019 and G0022 are Medicare community health integration (CHI) codes for certified or trained auxiliary personnel, such as community health workers, addressing social needs that affect a patient's care, under general supervision after an initiating visit. Health coaching codes are a separate service focused on coaching toward patient-set goals.

CHI codes were created in earlier Physician Fee Schedule rulemaking and appear on CMS's CY 2027 list of designated care management services assigned general supervision; the coaching codes do not (their general supervision comes from 42 CFR 410.26(b)(5)(iii)). A CHW might furnish CHI services, and, with appropriate coaching certification, coaching services, but the two must be documented and billed separately. Verify CHI requirements with CMS and your MAC.
