How is health coaching different from chronic care management (CCM)?

Answered by Angel Reyes, MCHES®Last verified: November 5, 2026

Short answer

CCM is monthly, time-based care management for patients with two or more chronic conditions and a comprehensive care plan. Health coaching (0591T–0593T) is per-session, face-to-face coaching toward patient-set goals. Both use general supervision, and the practitioner bills.

CMS valued 0591T and 0592T by crosswalking to complex CCM codes 99487 and 99489 (CMS-1848-F, p. 379), but that doesn't make coaching a CCM service. Read the full comparison: Chronic care management vs health coaching.
