What should a Medicare health coach job description include?
Short answer
The qualifying certification pathway and coaching-specific training, the supervising practitioner relationship, duties (initial, follow-up and group sessions; documentation with actual times; referral of clinical issues), and clear limits: no diagnosis, no triage, no billing.
Also include documentation expectations (individualized notes, no copy-forward), credential maintenance and exclusion screening, telehealth duties if applicable, and HIPAA training. Avoid compensation tied to Medicare referrals or volume without counsel review. The toolkit's practice integration kit includes a sample coach job description.
