Does the rule apply to Medicare Advantage or Medicaid?
Short answer
The rule sets payment under the Original Medicare Physician Fee Schedule. Medicare Advantage plans, Medicaid programs and commercial insurers set their own coverage and payment policies, so check each payer.
Some payers follow Medicare's lead and some do not. Never assume a commercial or Medicaid policy matches the CY 2027 Medicare rule.
