What is a supervision log, and is it required?
Short answer
A supervision log records who supervised, how the practitioner was available, and when charts were reviewed. The final rule doesn't require one, but it is a practical way to show general supervision is real.
The toolkit includes a supervision log template. Your organization's policy decides the format and frequency.
