Help center · Who furnishes and who bills
Can health coaches bill Medicare directly?
Short answer
No. Under the CY 2027 final rule, coaching is furnished by qualified auxiliary personnel incident to a practitioner's services, and only the supervising physician or other practitioner may bill Medicare (42 CFR 410.26(b)(5)(v)).
This applies to CHES®, MCHES®, NBC-HWC and nurse coaches alike. Nurse organizations asked CMS for direct billing and a supervision exemption; the final rule did not grant either (rule p. 380). The coach's role is to deliver and document the service so the practitioner's claim is supported.
