Help center · Who furnishes and who bills
Who submits the claim for 0591T–0593T?
Short answer
The supervising physician or other eligible practitioner. The regulation says only the supervising practitioner may bill Medicare for incident-to services, and the supervisor need not be the practitioner treating the patient more broadly.
See 42 CFR 410.26(b)(5)(iv) and (v), added by CMS-1848-F (display copy pp. 2059–2060). How the claim is built (rendering and billing provider fields, modifiers) is a question for your billing team and MAC.
