What does "incident to" mean for coaching?

Answered by Angel Reyes, MCHES®Last verified: November 5, 2026

Short answer

An incident-to service is furnished by auxiliary personnel as part of a physician's or other practitioner's services and billed by that practitioner. CMS placed health coaching in this framework under general supervision (42 CFR 410.26(b)(5)(iii)).

General incident-to conditions in 42 CFR 410.26 still apply, though the coaching section doesn't spell out how each applies to coaching. That is one reason to document the link to the practitioner's plan of care in every note. See What is general supervision?
