Can I use the same note text for every session?
Short answer
No. Copied-forward or cloned notes are a known audit concern. Templates should prompt you for fields, never supply the content; record what actually happened for each patient in each session.
A quick self-check: if you could swap the patient's name and the note would still be true, it is too generic.
