Does coaching need a practitioner order or plan of care?

Answered by Angel Reyes, MCHES®Last verified: November 5, 2026

Short answer

The final rule doesn't spell one out for coaching, but because coaching is an incident-to service, documenting the practitioner's order or plan of care that the coaching carries out is strongly advisable.

The toolkit's practitioner order and plan-of-care template covers diagnosis context, coaching goals, frequency guidance, signature and date.
