How should coaching time be documented?

Short answer

Record actual start and stop times for every session. 0592T and 0593T require at least 30 minutes; CMS's final table lists no time for 0591T, so follow the current AMA CPT® descriptor.

Never pre-fill or estimate times. For groups, record total group time and note anyone who arrived late or left early.
